I have implemented your recommendations and doubled the clickrate.
The experience is now much smoother and without any lags, the damage has increased significantly.

Now comes the big “but”:
Referring to the post by @TimothyLuke in Guide: macros, WoW, you and more, isn’t a clickrate of 50ms too obvious in the long run?
